Composition: 

Each capsule contains formoterol fumarate 12 mg as the active ingredient.
Excipients include lactose monohydrate and capsule base materials.

Product Description: 

Formohale is a long-acting β2-adrenergic agonist (LABA) bronchodilator.
It relaxes bronchial smooth muscle, improves airflow, and provides sustained relief of bronchospasm.
The product is supplied in capsule form for inhalation using the provided inhaler device.

Indications for Use: 

Asthma.
Chronic obstructive pulmonary disease (COPD).
Conditions requiring long-term bronchodilation.

Method of Administration and Dosage: 

Administer by inhalation only.
Place one capsule in the inhaler, pierce, and inhale the powder deeply through the mouthpiece.
Recommended dosage and frequency should follow physician’s prescription and package insert.

Contraindications: 

Hypersensitivity to formoterol or excipients.
Contraindicated in patients with severe cardiac arrhythmias or known intolerance to β2-agonists.

Precautions: 

Use with caution in patients with cardiovascular disease, diabetes mellitus, hyperthyroidism, or seizure disorders.
Not intended for relief of acute asthma attacks.
Concomitant use with other long-acting β2-agonists is not recommended.
