Properties:
Betamethasone is a synthetic GCS that has anti-inflammatory, antiallergic, antipruritic and vasoconstrictive effects. When applied topically, it constricts blood vessels, eliminates itching, reduces the release of inflammatory mediators (from eosinophils and mast cells), IL-1 and IL-2, γ-interferon (from lymphocytes and macrophages), inhibits the activity of hyaluronidase and reduces the permeability of the vascular wall. Interacts with specific receptors in the cytoplasm of the cell, stimulates the synthesis of mRNA, which induces the formation of proteins, incl. lipocortin mediating cellular effects. Lipocortin inhibits phospholipase A2, blocks the release of arachidonic acid and the biosynthesis of endoperoxides, PG, LT (which contribute to the development of inflammation, allergies and other pathological processes).
In allergic or traumatic lesions of the cornea, corticosteroids inhibit the proliferation of fibroblasts and post-inflammatory neovascularization of the cornea, thus maintaining its transparency.
Gentamicin is a broad-spectrum antibiotic of the aminoglycoside group. It has a bactericidal effect and provides a highly effective local treatment of primary and secondary bacterial infections of the skin. Active against many gram-positive and gram-negative bacteria, including Pseudomonas aeruginosa, Aerobacter aerogenes, Escherichia coli, indole-positive and indole-negative strains of Proteus vulgaris, Klebsiella pneumoniae, Enterobacter aerogenes, Serratia spp., Citrobacter spp., Salmonella spp., Shigella spp., Moraxella spp. . (including Moraxella lacunata), Neisseria spp. (including Neisseria gonorrhoeae), Streptococcus spp. (sensitive strains of β- and α-hemolytic group A streptococcus), Staphylococcus aureus (coagulase-positive, coagulase-negative strains and some penicillinase-producing strains).
Clioquinol is an antimicrobial agent for external use, a derivative of 8-hydroxyquinoline. Clioquinol is active against dermatophytes and yeast fungi (Microsporum spp., Trichophyton spp., Candida albicans) and gram-positive bacteria (Staphylococcus spp., Enterococcus spp.).
Indications:
When applied to the skin: dermatoses, allergic and inflammatory skin diseases amenable to GCS therapy, in the presence of a secondary infection caused by microorganisms susceptible to gentamicin, or if such an infection is suspected, incl. eczema (atopic, childhood, nummular), dermatitis (contact, allergic, seborrheic, atopic, solar, exfoliative, radiation), neurodermatitis, diaper rash, psoriasis, lichen planus, phlebatoderma, pyoderma, anogenital and senile itching.
Dosage and administration:
Apply 2-3 times a day. The course of treatment is 2-4 weeks.
Contraindications:
Hypersensitivity; when applied to the skin: skin tuberculosis, skin manifestations of syphilis, chicken pox, herpes simplex, fungal skin diseases; vaccination period and skin post-vaccination reactions; pregnancy (long-term treatment on large areas of the skin); lactation period; children’s age up to 6 months.
Precautionary measures:
When applied to the skin. If there is no effect from treatment within two weeks, it is recommended to contact your doctor to clarify the diagnosis and treatment.
If irritation or hypersensitivity is noted during the use of the combination, treatment should be discontinued and another therapy should be selected for the patient.
Any side effects of systemic corticosteroids, including suppression of the function of the adrenal cortex, can also be observed when using local corticosteroids, especially in children. Systemic absorption of local corticosteroids may increase with prolonged use, treatment of large body surfaces or the use of occlusive dressings. In such cases, especially in children, appropriate precautions should be observed, including the possibility of discontinuing the drug, determined by the doctor, depending on the severity of the disease.
Side effects:
– skin irritation (itching, erythema), burning sensation, dry skin, folliculitis, hypertrichosis, steroid acne, hypopigmentation, perioral dermatitis, allergic contact dermatitis, skin maceration, secondary infection, skin atrophy, striae, prickly heat.
When treating with gentamicin, transient skin irritation (erythema, itching) occurs, usually not requiring discontinuation of treatment.
Storage method:
At a temperature not higher than 30 degrees.
