Properties:

The mechanism by which phenytoin accelerates wound healing is unknown, animal and in vitro clinical studies suggest that phenytoin may be involved in the healing process at several levels, including stimulation of fibroblast proliferation, increased granulation tissue formation, decreased collagenase activity. A number of clinical studies show that phenytoin reduces the bacterial load on wounds. Phenytoin eliminates Staphylococcus aureus, E.Coli, Klebsiella spp. and pseudomonas spp. in infected wounds for 7 to 9 days in a guinea pig. Phenytoin has been found to be more conducive to healing of wounds infected with gram-negative organisms than gram-positive bacteria, it is not known whether phenytoin has antibacterial activity or whether phenytoin’s effect on bacterial load on wounds is mediated by effects on inflammatory cells and neovascularization. Local pain relief has also been observed with topical phenytoin therapy, which may be explained by its membrane stabilizing effect; Decreased inflammatory response may also facilitate nerve regeneration.

Indications:

Soft tissue wounds including pressure sores, diabetic foot ulcers, burns, traumatic wounds, venous congestive ulcers and abscesses. Topical phenytoin is also potentially useful in epidermolysis, bullosa simplex, and chronic skin ulcers.

Dosage and administration:

Apply a thin layer over the entire wound area and cover with gauze. Apply once a day.

Contraindications:

Patients with hypersensitivity to any of the active ingredients.

Precautionary measures:

Avoid contact with eyes. If there is sensitivity to the drug, it is necessary to cancel the use.

Side effects:

Side effects from phenytoin are rare. A generalized rash has also been reported, which disappeared after discontinuation of treatment. Hypertrophic granulation tissue was noted in (10-36) percent of patients in two studies. This can be corrected by discontinuing treatment, and it is suggested that discontinuing treatment when the wound area is covered with granulation matrix may prevent this effect.

Storage method:

Store at a temperature not exceeding 30 degrees.
