Compound:

Properties:

Combined drug for external use.

Calcipotriol is a synthetic analogue of the active metabolite of vitamin D. It inhibits the proliferation of keratinocytes and accelerates their morphological differentiation.

Betamethasone – GCS for external use; has a local anti-inflammatory, antipruritic, vasoconstrictor and immunosuppressive effect, however, the exact mechanisms of the antiinflammatory action of topical steroids are not fully understood.

Indications:

Chronic vulgar plaque psoriasis.

Method of administration and dosage:

Outwardly. Adults are applied to the affected skin 1 time / day.

The maximum daily dose is no more than 15 g; the maximum weekly dose is 100 g.

The recommended course of treatment is 4 weeks, further treatment under medical supervision.

Contraindications:

Diseases accompanied by impaired calcium metabolism; hypersensitivity to any component of the drug; severe liver failure; severe renal failure; viral (including herpes or chickenpox), fungal, bacterial and parasitic skin infections; rosacea; rosacea; acne vulgaris; perioral dermatitis; cutaneous manifestations of tuberculosis and syphilis; skin atrophy, skin ulcers; increased fragility of skin vessels; ichthyosis; perianal and genital itching; psoriatic erythroderma; teardrop-shaped, exfoliative, pustular psoriasis.

Precautions:

Not recommended for use on the face, mucous membranes and scalp. Do not apply to the periorbital area due to the possible development of cataracts and increased intraocular pressure. After applying the ointment, wash your hands thoroughly.

The risk of developing hypercalcemia when the recommended dosage regimen is observed is

minimal. However, when used in doses exceeding the recommended maximum weekly dose (100 g), hypercalcemia may develop, which quickly passes when the dose is reduced or canceled.

The drug contains a potent GCS, therefore it is not recommended to carry out concomitant treatment with other GCS.

It is recommended to avoid the use on large areas of the skin, making up more than 30% of the body surface, as well as under an occlusive dressing, to apply to the folds of the skin, as this increases the likelihood of systemic absorption of GCS and the development of systemic adverse reactions.

Side effects:

On the part of the skin and skin appendages: frequent – itching, rash, burning sensation; infrequent – pain, irritation, dermatitis, erythema, exacerbation of psoriasis, pigmentation disorders at the site of ointment application, folliculitis; rare – pustular psoriasis.

Long-term topical use of betamethasone (dipropionate) is associated with the development of skin atrophy, telangiectasias, stretch marks, folliculitis, hypertrichosis, perioral dermatitis, allergic contact dermatitis, depigmentation, colloidal degeneration of the skin, as well as an increased risk of developing generalized pustular photosensitivity, including hyper- and extremely rare cases of angioedema and facial edema.

Systemic reactions: associated with the use of calcipotriol (very rare): hypercalcemia or hypercalciuria. With prolonged use of local corticosteroids in high doses, it is possible to suppress the function of the adrenal glands with the development of secondary adrenal insufficiency and symptoms of hypercortisolism, including Itsenko-Cushing’s syndrome.
