Trade name:
Monuril
Composition:
Each sachet contains:
Fosfomycin 3 grams.
Auxiliary components:
sucrose; orange flavor; tangerine flavor; saccharin sodium.
Properties:
Broad-spectrum antibiotic derived from phosphonic acid. The mechanism of action is associated with the suppression of the first stage of the synthesis of the bacterial cell wall. Being a structural analogue of phosphoenolpyruvate, competitively irreversibly inhibits the enzyme UDP-N-acetylglucosamienolpyruvyltransferase, which catalyzes the formation reaction of UDP-M-acetyl-3-0- (1-carboxyvinyl) -0-glucosamine from phosphoenolpyruvate and UDP-D-N-acetyl amine … Also fosfomycin is able to reduce the adhesion of bacteria to the mucous membranes of the bladder, which can play the role of a predisposing factor for recurrent infections.
Fosfomycin is active against gram-positive aerobes. Secondary resistance of microorganisms to fosfomycin is rare. There is no cross-resistance with other antibacterial agents. When combined with beta-lactams, fluoroquinolones, glycopeptides or aminoglycosides, there is a pronounced synergism of antimicrobial action against a wide range of gram-positive and gram-negative aerobic bacteria.
Indications:
Bacterial urinary tract infections of various localization caused by microorganisms sensitive to fosfomycin:
– acute uncomplicated urinary tract infections; asymptomatic bacteriuria;
– prevention of urinary tract infections after surgery and transurethral diagnostic studies.
Method of administration and dosage:
Inside.
Monuril is used once, on an empty stomach, 2-3 hours before or after a meal, preferably before bedtime, after having emptied the bladder: for adults and children over 12 years old in a daily dose of 3 g. The contents of one package are dissolved in about 150-200 ml of water and take immediately after preparation.
The course of treatment is 1 day.
For the prevention of urinary tract infections during surgery, diagnostic procedures, the indicated dose is taken 2 times – 3 hours before the intervention and 24 hours after it.
Contraindications:
Not for use by people weighing less than 50 kg.
Hypersensitivity to fosfomycin or other components of the drug; severe renal failure.
Precautionary measures:
Acute and persistent diarrhea during or after antibiotic treatment can be a symptom of antibiotic-induced colitis. The most serious form is pseudomembranous colitis.
The preparation contains sucrose. Patients with rare hereditary diseases, fructose intolerance, or sucrase-isomaltase deficiency should not take this drug.
Simultaneous food intake slows down the absorption of granules, therefore, it is recommended to take the drug on an empty stomach, 2-3 hours before or after a meal.

On the part of the genitals and mammary gland: Not frequent: vulvovaginitis.
Blood and lymphatic system disorders: Not frequent: thrombocytosis, leukopenia.
