Description: 

Isoptin SR is a calcium channel blocker of the phenylalkylamine group.
It lowers blood pressure, treats angina pectoris, and controls supraventricular arrhythmias.
Its mechanism of action involves relaxation of vascular smooth muscle and slowing of AV conduction.

Indications for Use: 

Management of arterial hypertension.
Treatment of stable and unstable angina pectoris, including vasospastic angina.
Control of supraventricular tachyarrhythmias such as paroxysmal supraventricular tachycardia and atrial fibrillation/flutter with rapid ventricular response (except in patients with WPW or Lown‑Ganong‑Levine syndromes).

Method of Administration and Dosage: 

• The initial dose is usually 180 mg once daily in the morning with food.
  
• Depending on patient response, the dose may be increased to 240 mg daily, or divided into morning and evening doses, up to a maximum of 480 mg/day. 
  
• Tablets must be swallowed whole with water, not grapefruit juice, and must not be chewed or crushed.

Contraindications: 

• Hypersensitivity to verapamil or excipients.
  
• Severe hypotension. 
  
• Cardiogenic shock. 
  
• Advanced AV block or sick sinus syndrome without pacemaker. 
  
• Heart failure with reduced ejection fraction. 
  
• Atrial fibrillation/flutter with accessory pathways (WPW, LGL). 
  
• Concomitant use with ivabradine. 
  
• Simultaneous IV administration with beta‑blockers.

Precautions: 

• Use with caution after recent myocardial infarction, especially if complications are present.
  
• May cause or worsen conduction disorders or heart failure. 
  
• Monitor patients with systolic BP <90 mmHg. 
  
• Dose adjustment is required in hepatic impairment. 
  
• Verapamil potentiates digoxin, beta‑blockers, antiarrhythmics, and statins. 
  
• Each tablet contains ~37 mg sodium, relevant for patients on low‑sodium diets.

Adverse Reactions: 

Common effects include constipation, dizziness, headache, bradycardia, hypotension, peripheral edema, fatigue, nausea, and abdominal pain.
Less common but serious effects include AV block, heart failure, hepatic enzyme elevations, allergic reactions, gingival hyperplasia, and rare cases of Stevens‑Johnson syndrome or galactorrhea.

Pregnancy and Lactation: 

Use only when clearly necessary after benefit–risk assessment.
Verapamil is excreted in breast milk;
use with caution.

Storage Conditions: 

Store below 30°C in the original package, protected from light and moisture.
Keep out of reach of children.
