Composition:
Each tablet contains:
Propafenone hydrochloride 150 mg
Inactive ingredients:
lactose monohydrate; MCC; sodium lauryl sulfate; sodium starch glycolate; povidone; talc; magnesium stearate; Opadray white Y-1-7000.
Properties:
Class IC antiarrhythmic agent, sodium channel blocker. Reduces the maximum rate of depolarization of phase 0 of the action potential and its amplitude mainly in the Purkinje fibers and contractile fibers of the ventricles, reduces automatism. Highly effective in ventricular arrhythmias; with supraventricular arrhythmias, the effectiveness is somewhat lower. Propafenone has a mild beta-adrenergic blocking effect.
Indications:
Treatment and prevention of supraventricular and ventricular extrasystoles, paroxysmal arrhythmias (supraventricular tachycardia, including with WPW syndrome; ventricular tachycardia).
Dosage and administration:
The optimal dosing regimen is determined by the doctor. Compliance of the used dosage form of a particular drug with indications for use and dosing regimen should be strictly observed.
When administered orally, the initial dose is 450-600 mg / day, if necessary, the dose is increased to 900 mg / day.
Contraindications:
Severe forms of chronic heart failure, severe arterial hypotension, cardiogenic shock, severe bradycardia, SSS, II and III degree AV blockade, electrolyte imbalance, myasthenia gravis, severe obstructive pulmonary disease, hepatic cholestasis, childhood, hypersensitivity to propafenone.
Warnings and precautions:
Use with caution in violations of liver function, severe violations of kidney function, as well as in combination with other antiarrhythmic drugs with similar electrophysiological parameters.
Treatment should be started in a hospital setting, as the risk of arrhythmogenic effects associated with the use of propafenone is increased. It is recommended that prior antiarrhythmic therapy be discontinued prior to initiation of propafenone treatment within 2-5 T1/2.
Propafenone is characterized by pronounced individual differences in the values of the concentration of the active substance in the blood plasma, so careful selection of doses for each patient is recommended.
Side effects:
From the side of the cardiovascular system: possible bradycardia, slowing of sinoatrial, AV and intraventricular conduction, decreased myocardial contractility (in predisposed patients), arrhythmogenic effect; when taken in high doses – orthostatic hypotension.
From the digestive system: when taken in high doses, nausea, anorexia, a feeling of heaviness in the epigastrium, constipation are possible; rarely – abnormal liver function.
From the side of the central nervous system: when taken in high doses – headache, dizziness; rarely – visual impairment.
From the hematopoietic system: leukopenia, agranulocytosis, thrombocytopenia.
From the reproductive system: oligospermia.
Allergic reactions: rarely – skin rash.
Storage:
In a dry cool place at a temperature not exceeding 30 °C.
