Properties:
A second generation slow calcium channel blocker (BMCC), a dihydropyridine derivative. It has antianginal and hypotensive effects. By binding to dihydropyridine receptors, it blocks calcium channels, reduces the transmembrane transition of calcium ions into the cell (to a greater extent in vascular smooth muscle cells than in cardiomyocytes).
The antianginal effect is due to the expansion of the coronary and peripheral arteries and arterioles: with angina pectoris, it reduces the severity of myocardial ischemia; expanding the peripheral arterioles, reduces the systemic vascular resistance; reduces preload on the heart, myocardial oxygen demand. Expands the main coronary arteries and arterioles in unchanged and ischemic zones of the myocardium, increases the flow of oxygen to the myocardium (especially with vasospastic angina); prevents the development of spasm of the coronary arteries (including those caused by smoking). In patients with angina pectoris, a single daily dose increases exercise tolerance, delays the development of the next attack of angina pectoris and ischemic depression of the ST segment; reduces the frequency of angina attacks and nitroglycerin consumption.
Amlodipine has a long-term dose-dependent hypotensive effect, which is due to a direct vasodilating effect on vascular smooth muscle. With arterial hypertension, a single daily dose of amlodipine provides a clinically significant decrease in blood pressure for 24 hours (with the patient lying and standing).
Reduces the degree of left ventricular myocardial hypertrophy, has anti-atherosclerotic and cardioprotective effects in coronary artery disease. It has no effect on myocardial contractility and conductivity, inhibits platelet aggregation, increases GFR, and has a weak natriuretic effect. In diabetic nephropathy, it does not increase the severity of microalbuminuria. Does not adversely affect metabolism and plasma lipid concentration.
Indications:
– arterial hypertension (as monotherapy or in combination with other antihypertensive drugs);

Method of administration and dosage:
The drug is taken orally, regardless of food intake.
For the treatment of arterial hypertension and the prevention of attacks of exertional angina and vasospastic angina, the initial dose is 5 mg 1 time / day. If necessary, the daily dose can be increased to a maximum of 10 mg (1 time / day).
Contraindications:
– severe arterial hypotension (systolic blood pressure less than 90 mm Hg);

-cardiogenic shock;
– unstable angina (except for Prinzmetal’s angina);
– Severe aortic stenosis;
– lactose intolerance, lactase deficiency or glucose / galactose malabsorption syndrome;

Precautionary measures:
During the period of drug treatment, it is necessary to control the patient’s body weight and the amount of sodium chloride consumed; prescribe an appropriate diet with limited salt intake.
Maintaining dental hygiene and regular visits to the dentist is necessary (to prevent soreness, bleeding and gingival hyperplasia).
The dosage regimen of the drug in elderly patients is similar to that in patients of other age groups. When the dose is increased, careful monitoring of elderly patients is necessary.
Side effects:
From the side of the cardiovascular system: often – a feeling of palpitations, peripheral edema (swelling of the ankles and feet).
From the side of the central nervous system: often – headache, dizziness, fatigue.
