Properties:

Combined drug, has a hypotensive and diuretic effect.

Captopril is an ACE inhibitor. Reduces the formation of angiotensin II from angiotensin I, reduces the release of aldosterone. Reduces blood pressure, OPSS, afterload, preload. Expands arteries more than veins. Enhances coronary and renal blood flow. With prolonged use, it reduces the severity of myocardial hypertrophy and the walls of resistive arteries; improves blood supply to ischemic myocardium; reduces platelet aggregation.

Hydrochlorothiazide is a medium-strength thiazide diuretic that reduces the reabsorption of sodium ions at the level of the cortical segment of the loop of Henle. Does not affect the acid-base state. It reduces blood pressure by changing the reactivity of the vascular wall, reducing the pressor effect of vasoconstrictor substances (epinephrine, norepinephrine) and increasing the depressant effect on the autonomic ganglia (to a lesser extent, due to a decrease in BCC). Enhances the hypotensive effect of captopril.

Indications:

Arterial hypertension (patients who are indicated for combination therapy).

Dosage and administration:

The optimal dosing regimen is determined by the doctor. Compliance of the dosage form of a particular drug with indications for use and dosing regimen should be strictly observed.

It is taken orally at a dose of 50 mg of captopril + 25 mg of hydrochlorothiazide 1 time/day.

Contraindications:

Angioedema hereditary or idiopathic (in history while taking ACE inhibitors);

– stenosis of the aortic mouth;

- mitral stenosis;

– hypertrophic obstructive cardiomyopathy;

– bilateral stenosis of the renal arteries or stenosis of the artery of a single kidney;

– condition after kidney transplantation;

– chronic heart failure;

- cardiogenic shock;

- arterial hypotension;

– hepatic insufficiency of severe degree (precomatous state, hepatic coma);

– primary hyperaldosteronism;

– hypersensitivity to the components of the combination, other ACE inhibitors, thiazide diuretics, sulfonamide derivatives (cross-allergic reactions are possible).

Precautionary measures:

At the beginning of treatment, an excessive decrease in blood pressure may occur, especially in patients with chronic heart failure, severe arterial hypertension (including renal origin) and / or renal failure. Before starting treatment, it is necessary to compensate for the deficiency of sodium ions and bring the BCC back to normal (reduce the dose of previously prescribed diuretics or, in some cases, completely cancel them), as well as determine indicators of kidney function.

It is necessary to regularly monitor the concentration of potassium and calcium in the blood serum (especially in patients receiving treatment with cardiac glycosides, glucocorticoids, often using laxatives, as well as in elderly patients), glucose, uric acid, lipids (cholesterol and triglycerides), urea and creatinine, activity hepatic enzymes.

Side effects:

From the respiratory system: often – dry unproductive cough, shortness of breath; very rarely – bronchospasm, eosinophilic pneumonitis, rhinitis, pulmonary edema.

Allergic reactions: often – skin itching, with or without rashes, sometimes accompanied by fever and arthralgia, skin rashes.

From the side of the central nervous system: often – drowsiness, dizziness; rarely – headache, ataxia, paresthesia.

From the digestive system: often – nausea, vomiting, irritation of the gastric mucosa, pain in the abdominal cavity, diarrhea, constipation, taste disturbance, dryness of the oral mucosa.
