Composition:
Each 5 ml contains:
Salbutamol sulphate 2.4 mg (equivalent to 2 mg salbutamol)
Ammonium chloride 100 mg.
Inactive ingredients:
Riboflavin sodium phosphate, Sodium metabisulphite, Disodium edetate, Propylparaben, Methylparaben, Sodium citrate, Anhydrous citric acid, Propylene glycol, Sorbitol 70%, Sucrose, Mandarin oil, Purified water.
Description:
Farcolin is a bronchodilator, antiasthmatic, and expectorant combination syrup.
Salbutamol, a selective β2-adrenergic agonist, relaxes bronchial smooth muscles, resulting in bronchodilation, decreased airway resistance, and relief of dyspnea.
Ammonium chloride acts as an expectorant, facilitating mucus clearance from the respiratory tract.
Indications for Use:
As a bronchodilator and expectorant for the chronic management or prophylaxis of bronchial asthma, and for reversible bronchospasm associated with bronchitis and emphysema.
Dosage and Administration:
- Adults: One teaspoonful (5 ml) three times daily; may be increased to two teaspoonfuls three to four times daily.
- Children (4-6 years): Half a teaspoonful three times daily; may be increased to one teaspoonful three to four times daily.
- Children (6-12 years): One teaspoonful three times daily; may be increased to two teaspoonfuls three to four times daily.
- Farcolin Syrup is generally well tolerated in children; doses may be cautiously increased if necessary.
Contraindications:
Hypersensitivity to the active ingredients or any excipients.
Precautions:
- Bronchodilators should not be the only treatment in severe or unstable asthma.
- Increased use of β2-agonists indicates deterioration in asthma control and requires medical reassessment.
- Caution in patients with cardiovascular disorders (ischaemic heart disease, arrhythmia, heart failure), thyrotoxicosis, or diabetes.
- Potential hypokalaemia may occur, particularly with concomitant xanthine derivatives, steroids, or hypoxia - monitor serum potassium levels.
- Contains sucrose and sodium metabisulphite, which may cause allergic or anaphylactic reactions, especially in asthmatics.
- Each 5 ml contains 19.7 mg sodium - consider in low-sodium diets.
Side Effects:
Very common - tremor.
Common - headache, tachycardia, palpitations, muscle cramps.
Rare - cardiac arrhythmias, peripheral vasodilation, hypokalaemia.
Very rare - hyperactivity, muscle tension, hypersensitivity reactions (angioedema, urticaria, bronchospasm, hypotension, collapse).
Unknown - myocardial ischaemia.
Overdose:
Symptoms include tremor, tachycardia, hyperactivity, nausea, vomiting, hypokalaemia, and hyperglycaemia.
Monitor serum potassium and acid-base balance.
Excess ammonium chloride may cause nausea and vomiting; symptomatic management is usually sufficient.
Pregnancy and Lactation:
Use during pregnancy only if the expected benefit outweighs potential risks to the fetus.
As salbutamol is likely excreted in breast milk, use during breastfeeding requires caution.
Storage Conditions:
Store below 30°C, protected from light.
Keep out of reach of children.
