Compound:

Each tablet contains:

Irbesartan 300 mg

Auxiliary components:

Microcrystalline cellulose, croscarmellose sodium, lactose monohydrate, magnesium stearate, colloidal silicon dioxide, starch, povidone.

Properties:

Antihypertensive agent, blocker of angiotensin II (type AT1) receptors.

Reduces the concentration of aldosterone in plasma (does not inhibit kinase II, which destroys bradykinin); eliminates the vasoconstrictor effect of angiotensin II; reduces peripheral vascular resistance, reduces afterload, systemic blood pressure and pressure in the pulmonary circulation. Does not affect the concentration of triglycerides, cholesterol, glucose, uric acid in plasma and the excretion of uric acid.

Indications:

-essential hypertension;

– nephropathy in arterial hypertension and type 2 diabetes mellitus (as part of combined antihypertensive therapy).

Dosage and administration:

The drug should be taken orally, the tablet is swallowed whole with water. The initial and maintenance dose is 150 mg 1 time / day, regardless of the meal. The use of the drug at this dose provides more optimal 24-hour blood pressure control than at a dose of 75 mg / day. However, in some cases, especially in patients on hemodialysis or in patients over the age of 75 years, the initial dose should be 75 mg.

With insufficient therapeutic effect when using the drug at a dose of 150 mg 1 time / day, the dose can be increased to 300 mg, or another antihypertensive agent should be prescribed. In particular, administration of a diuretic such as hydrochlorothiazide has been shown to enhance the effects of irbesartan.

Contraindications:

-pregnancy;

– lactation period;

-children and adolescents under 18 years of age (efficacy and safety have not been established);

– lactose intolerance, lactase deficiency and glucose-galactose malabsorption syndrome;

– Hypersensitivity to the components of the drug.

Precautionary measures:

Treatment should be carried out under the control of blood pressure.

In dehydrated patients, as well as in sodium deficiency (as a result of intensive treatment with diuretics, diarrhea or vomiting, restriction of salt intake with food) and in patients on hemodialysis, symptomatic arterial hypotension may develop, especially after taking the first dose of the drug.

If the water and electrolyte balance is disturbed, the volume of circulating blood should be restored before the start of the drug.

In patients with renal insufficiency, periodic monitoring of the concentration of potassium and creatinine in the blood plasma should be carried out.

Side effects:

From the side of the central nervous system: often – dizziness; very rarely – headache.

From the digestive system: often – nausea, vomiting.

On the part of the body as a whole: often – increased fatigue; infrequently – pain in the chest.

On the part of laboratory parameters: often – a significant increase in the activity of creatine phosphokinase (CPK) (1.7%), not accompanied by clinical manifestations of the musculoskeletal system.

Arterial hypertension and type 2 diabetes mellitus with kidney damage:

From the nervous system: often – orthostatic dizziness.

Since the cardiovascular system: often – orthostatic hypotension.

From the musculoskeletal system: often – pain in the muscles and bones.
