Composition:
Each tablet contains:
Vildagliptin 50 mg
Metformin hydrochloride 850 mg.
Description:
Combined oral antidiabetic agent containing vildagliptin (DPP-4 inhibitor) and metformin (biguanide). Used for blood glucose control in type 2 diabetes mellitus when metformin monotherapy or combination with other agents is insufficient.
Indications:
- Type 2 diabetes mellitus in adults:
- Combined with diet and exercise.
- As add-on to sulfonylurea or insulin when glycemic control is inadequate.
Dosage and Administration:
Standard dose: 1 tablet twice daily with meals.
Dose adjustment: Based on renal function and concomitant therapy.
Renal impairment: Dose adjustment required (contraindicated if Cl cr < 60 mL/min).
Elderly: Dose adjustment based on renal function.
Important:
Swallow whole with water, do not chew.
Do not exceed maximum daily dose: 100 mg vildagliptin / 2000 mg metformin.
Contraindications:
- Hypersensitivity to vildagliptin, metformin, or excipients.
- Severe renal impairment (Cl cr < 60 mL/min).
- Diabetic ketoacidosis, type 1 diabetes.
- Heart failure, recent myocardial infarction.
- Severe infections, dehydration, alcoholism.
- Pregnancy, lactation.
- Children under 18 years.
Precautions:
Before treatment:
Assess renal and hepatic function.
Test glucose, HbA1c, vitamin B12 levels.
During treatment:
Regular liver function monitoring (every 3 months in first year).
Renal function monitoring (annually, more frequently in elderly).
Vitamin B12 levels (every 2–3 years).
During radiocontrast procedures:
Discontinue 48 hours before and resume 48 hours after the procedure.
Adverse Effects:
Common (>1/100):
Dizziness, nausea, diarrhea, abdominal pain.
Hypoglycemia (when combined with sulfonylurea or insulin).
Rare (<1/1000):
Allergic reactions (angioedema, rash).
Liver dysfunction (elevated ALT, AST).
Lactic acidosis (rare but life-threatening).
Symptoms of lactic acidosis:
Muscle pain, weakness, difficulty breathing, nausea, vomiting.
Requires immediate medical attention!
Pregnancy and Lactation:
Pregnancy: Contraindicated. Use insulin.
Lactation: Not recommended (no data on excretion in milk).
Storage:
Store below 30°C in a dry place.
