Composition:

Each pre-filled syringe contains:
Enoxaparin sodium 6000 IU (60 mg), water for injection (excipient).

Product Description:

Clexane is a low molecular weight heparin (LMWH) anticoagulant.
It works by inhibiting clotting factors, particularly Factor Xa and thrombin, thereby preventing the formation of harmful blood clots.
Anticoagulant effect via Factor Xa inhibition.
Prevents thrombus formation in veins and arteries.
Reduces risk of clot-related complications in surgery and acute coronary syndromes.

Indications for Use:

Prophylaxis of venous thromboembolism (VTE) in surgical patients (orthopedic, general, cancer surgery).
Prophylaxis of VTE in medical patients with acute illness and reduced mobility.
Treatment of DVT and PE (excluding PE requiring thrombolysis or surgery).
Prevention of thrombus formation during hemodialysis.
Treatment of acute coronary syndromes (unstable angina, NSTEMI, STEMI) in combination with aspirin.

Method of Administration and Dosage:

Moderate risk surgical prophylaxis: 2000 IU (20 mg) SC daily, first dose 2 hours before surgery.
High risk surgical prophylaxis: 4000 IU (40 mg) SC daily, preferably 12 hours before surgery.
Medical patients with acute illness: 4000 IU (40 mg) SC daily for 6–14 days.
Treatment of DVT/PE: 1 mg/kg SC every 12 hours or 1.5 mg/kg SC daily for ~10 days, with transition to oral anticoagulant.
Haemodialysis: 1 mg/kg SC; reduced dose for high bleeding risk.
Acute coronary syndromes: 1 mg/kg SC every 12 hours with aspirin;
STEMI patients may require IV bolus + SC dosing.

Contraindications:

Active major bleeding.
History of heparin-induced thrombocytopenia (HIT).
Hypersensitivity to enoxaparin, heparin, or pork products.
Recent spinal/epidural anaesthesia with risk of spinal hematoma.
Severe uncontrolled hypertension, active peptic ulcer, or recent brain/spine/eye surgery.

Precautions:

Monitor for bleeding risk, especially in elderly or renal impairment.
Use with caution in patients with liver disease, uncontrolled hypertension, or diabetic retinopathy.
Avoid concurrent use with other anticoagulants unless transitioning therapy.

Adverse Reactions:

Common: bleeding, hematomas at injection site, anaemia, nausea.
Rare: allergic reactions, heparin-induced thrombocytopenia (HIT), spinal hematoma (risk with epidural anaesthesia).

Pregnancy and Lactation:

May be used under medical supervision; caution in patients with mechanical heart valves.
Limited data in breastfeeding; consult physician.

Storage Conditions:

Store at room temperature, protected from light and moisture.
